Many people assisted in the production of this book and I am grateful to them all. I would particularly like to thank Professor Ann Sommerville, Head of Medical Ethics, and Dr Caroline Seddon, Head of Science and Education, at the British Medical Association, for their kind permission to reproduce various items from their reports, at Appendix 1, as examples of current best practice.
